DENTAL HYGIENIST

  1. GRADUATE OF TWO YEARS CERTIFICATE IN DENTAL HYGIENE CONFERRED BY A RECOGNIZED COLLEGE
  2. NSO / PSA Birth Certificate
  3. NSO / PSA Marriage Contract (for married female applicants)
  4. Transcript of Records with scanned picture and Remarks “For Board Examination Purposes”
  5. Valid NBI Clearance
  6. One (1) Good Moral Certificate from any of the following:
  • Barangay
  • Church
  • School
  • Employer
  1. Diploma
  2. OJT Certificate / Employment Certificate (6 months)
  3. NC Certificate from TESDA

After three (3) failures: REFRESHER COURSE

Graduate of two (2) years certificate in dental hygiene conferred by a recognized college or university
