- BS in Metallurgical Engineering
- NSO / PSA Birth Certificate
- NSO / PSA Marriage Contract (for married female applicants)
- Transcript of Records with scanned picture and Remarks “For Board Examination Purposes”
- Valid NBI Clearance (First Timers & Repeaters)
- One (1)Certificate of Good Moral Character from any of the following:
- Barangay
- School
- Church
- Employer
A citizen of the Philippines or a foreign citizen whose country has reciprocity agreement with the Philippines
In case of foreign applicants:
a. Proof of reciprocity which clearly expresses the provision of the law of his country/state or international treaty, agreement and/or covenant to which his/her country or state is a signatory, that allows Filipino citizens to take licensure examinations, if any, and to register as Metallurgical Engineers in his/her country or state on terms of strict and absolute equality with the citizens, subjects or nationals of said country or state including the unconditional recognition of prerequisite degrees issued by institutions of higher learning duly recognized or established by the Government of the Republic of the Philippines, attaching/appending thereto an authentic or authenticated office copy of said law or treaty, agreement or covenant officially translated in the English language.
b. The original or certified copy of any official document issued by the Bureau of Immigration allowing the applicant to enter and reside in the Philippines;
c. Photocopy of passport; and
d. Original or authenticated copy of transcript of records or equivalent document of the course for licensure examination issued by the institution of higher learning where he studied, duly authorized or accredited by his country or state;
Bachelor of Science in Metallurgical Engineering, Metallurgy or related engineering degree majoring in Metallurgical Engineering or Metallurgy from a university, school, college, academy or institute duly constituted and recognized as such by the governent.
Conditioned/Removal Examination:
To be taken within one (1) year from date of release of results, and if the examinee still fails to obtain a passing grade of 75%, he/she has to take the complete examination.
