  1. BS in Respiratory Therapy
  2. NSO / PSA Birth Certificate NSO / PSA Marriage Contract (for married female applicants)
  3. Transcript of Records with scanned picture and Remarks “For Board Examination Purposes”
  4. Payment: P900.00 - complete Valid NBI Clearance (First Timers & Repeaters)
  5. Certification of ten (10) months of internshipeequivalent to a minimum of one thousand five hundred (1500) hours of Respiratory therapy clinical internship under the supervision of a duly qualified and registered respiratory therapists.
  6. Three (3) certificate of Good Moral Character from any of the following and duly signed by by the issuing authority and duly notarized under oath:
  • Barangay
  • School
  • Church
  • Employer
  1. CAV (Certificate of Authentication and Verification) for those school without Special Order (SO) number
