  1. BS IN SANITARY ENGINEERING / BS IN CIVIL ENGINEERING HAVING TAKEN MAJOR SUBJECTS IN SANITARY ENGINEERING
  2. NSO / PSA Birth Certificate
  3. NSO / PSA Marriage Contract (for married female applicants)
  4. Transcript of Records with scanned picture and Remarks “For Board Examination Purposes”
  5. Three (3) Certificate of Good Moral Character from any of the following:
  • Parent
  • School
  • Barangay
  • Office of occupation or supervisor

After three (3) failures - REFRESHER COURSE (FTB)

  • If Registered Civil Engineer provide photocopy of valid PRC license*
