  4. Transcript of Records with scanned picture, S.O. No. (unless exempted) and Reamarks “For Board Examination Purposes”
  5. Original and photocopy of National Bureau of Investigation (NBI) Clearance

Certificate of 1,000 case hours of supervised field practice signed by faculty supervisor and/or head of the social work program, with updated Professional Identification Card (PIC)
