LOST DRIVER’S LICENSE Step-by-step for Duplicate Copy:
•Get a Notarized ‘Affidavit of Loss’ before going to LTO and pumila para kumuha ng Form
(normally meron nito malapit or same place kung nasaan ang LTO Office). Unahin muna po makakuha nito.
₱200 po yung binayad ko,
• Mag pa-xerox na ng 2 Valid IDs (Gov’t Issued). Dalhin pa rin yung mga actual or Original IDs na ito, kasi ipapakita sa Assessment/Receiving for License Application/Renewal, matic na yan na meron din Xerox na nasa tabi o harap lang ng LTO, 
• Sa pila ng License, bibigyan ka ng form kapag naipakita mo na sa kanila yung IDs mo na may kasamang xerox copy, attached na rin nila yung Affidavit of Loss,
• After ma fill-out-an yung form, babalik dun sa nag assessment, para mabigyan ng number for Encoding and makapagbayad sa Cashier,
• After makapagbayad, (please check the O.R below, ₱472.63) Next ay tatawagin for new picture taking, finger prints and signature,
• Last, printing and releasing of your Duplicate Driver’s License, approximate waiting time: 5-10 minutes after nung sinundan na step.
TOTAL COST: ₱672.63 plus ₱6 xerox
Medyo may kamahalan po for me, at syempre pag lalaanan na naman ng time para makapunta sa LTO. Kaya ingatan po natin ang ating lisensya.

Konti lang po ang tao sa branch nila kanina.
Mababait pa po sila, dami ko po kasing tanong. Pero very accommodating naman po sila sa pag-sagot.
